McDonald's has been hit by a Happy Meals promotion in the United States and Canada that has gone disastrously wrong .
The burger chain is recalling about 234,000 Scooter Bug toys which were distributed with its Happy Meals for the under-3s.
The company received two reports of children choking and one of a child gagging on the broken off antenna.
The promotion ran for four months from November.
